Clergy Sample W-2 Form Preparer

This worksheet may serve to simplify the preparations of Form W-2 for Ministers. Fill out the forms that apply, and click the "COMPLETE" button. The sample W-2 form at the bottom of the page will automatically be filled out. Print out this page, and use it as a guideline for filling out the real W-2. The W-2 created here is NOT to be sent to the IRS!

    For most clergy, be sure to leave the following boxes blank: a, 3, 4, 5, 6, 7, 8, 9, 10, and 11. The treasurer needs to read the IRS instructions and possibly seek professional advice before entering any amount in those boxes for a minister.

    Send W-2's to all employees by February 1 and mail red copies to Social Security by February 28 wih Form W-3. If mistakes are discovered, they can be corrected later using Form W-2c and W-3c.
